Wellness Incentive Program

The <strong>FOCUS</strong><em>fwd</em> Wellness Incentive Program is designed to help members lead a healthier lifestyle.

By completing health-related activities and challenges, your clients can earn up to $110 in rewards and increase their chances of winning one of the $1,000 quarterly and $5,000 annual cash rewards in our Sweepstakes.

  • FOCUS Points*

    Members can earn money and sweepstakes entries when they complete activities that are important to improving their overall health.

    Members get a $70 reward and 40 Sweepstakes entries when they complete the following activities: personal health assessment, annual wellness exam, and preventive screening or flu vaccine.

    *This is a calendar-year program and will restart annually.

  • GET FIT*

    Members receive $10 in rewards and 10 Sweepstakes entries for each challenge they complete, for a total of $40 in rewards and 40 Sweepstakes entries each calendar year.

    The GET FIT quarterly challenge rewards members for taking steps toward their exercise goals — an average of 5,500 steps per day to be exact. Now with a new challenge every three-months, it's never been easier to get started.

    They'll receive $10 in rewards and 10 Sweepstakes entries for each challenge they complete, for a total of $40 in rewards and 40 Sweepstakes entries each calendar year.

    To complete the GET FIT quarterly challenge, they can connect a fitness tracker to their account and get moving!

  • Sweepstakes

    Members earn entries into the Sweepstakes for chances to win one of the three $1,000 quarterly and three $5,000 annual cash rewards.

    When members complete FOCUS Points and GET FIT, they get 80 entries into the Sweepstakes, increasing their chances to win one of the three $1,000 quarterly and three $5,000 annual cash rewards. Simply signing up for FOCUSfwd and completing any of its programs can give members rewards and more Sweepstakes entries.

    Members can sign up from their My Health Toolkit account.

    To get started:

    • Access the FOCUSfwd Wellness Incentive Program from their My Health Toolkit® account to get registered.
    • They'll need to enter their email address to be eligible to win one of the Sweepstakes rewards!

    If they don't have a My Health Toolkit account, it only takes a few minutes to create one. Once logged in, they can choose which task they would like to complete first.

If a member has previously downloaded the app, they can follow these steps for an improved experience. 

 

Instructions for Apple users:

  1. Uninstall the app by pressing and holding down the app icon and selecting "Remove App", then "Delete App" from the menu.
  2. Reinstall the app by opening the App Store, searching for the FOCUSfwd app and then reinstalling. 
  3. Once reinstalled, log in with existing credentials. 

 

Instructions for Android users:

  1. Uninstall the app by going to Settings > Apps > FOCUSfwd > Uninstall.
  2. Reinstall the app by going to the Google Play Store, searching for the FOCUSfwd app and then reinstalling.
