My Diabetes Discount Program can help.

My Diabetes Discount Program allows your covered employees with Type 1 or Type 2 diabetes who are actively on insulin therapy to receive their insulin with a $0 copay.*

How it works:

If eligible, they’ll need to complete specific requirements to begin receiving their $0 insulin copay. Program requirements are evaluated on a quarterly basis. Their $0 insulin copay will begin the quarter after they complete all requirements and will remain active for one year. To continually receive their $0 insulin copay, they must maintain the program requirements each year.

Your covered employees should contact Member Services using the number on the back of their ID card to:

• Receive a detailed checklist of program requirements.
• Get connected to a care manager to see which requirements they still need to meet.
