Agents can request a copy of 1095-B forms to be sent to the group by: 

• Contacting their Renewal Account Executive.
• Emailing BCHPSmall@BlueChoiceSC.com.
   

1094-B and 1095-B forms are submitted to the Internal Revenue Service (IRS) on the members’ behalf.*
 

Individual Coverage

Members access their 1095-B forms by logging in to My Health Toolkit.
 

*Groups with 51+ employees are responsible for submitting 1094-C and 1095-C forms to the IRS.
